Outreach and Engagement
Outreach and engagement are the gateway to health equity and one of the most powerful tools for reaching vulnerable, under-resourced, and underserved populations. Across Nevada, many individuals face barriers that prevent them from accessing healthcare, housing, behavioral health services, substance use treatment, benefits, and other essential resources. Community Health Workers serve as trusted messengers and boots-on-the-ground connectors who build relationships, identify unmet needs, and connect individuals to life-changing services. Harmony Health Integration leverages outreach and engagement to bridge communities with healthcare systems, public health initiatives, research institutions, hospitals, universities, and community partners. Through culturally responsive engagement, we help organizations reach populations that are often overlooked, while empowering individuals to access resources, improve health outcomes, and overcome barriers related to the social determinants of health.
