Purpose of HHI is
to Assist those who need assistance.
Harmony Health Integration was created to meet individuals at the intersection of need, with a model that is grounded in trust, cultural responsiveness, and community connection. At the center of this work are Community Health Workers (CHWs)—trusted individuals who serve as navigators, advocates, and bridges between systems and the people they are meant to serve. We are dedicated to advancing whole-person care and ensuring no one navigates complex systems alone.

Population of Focus
At the intersection of need, Harmony Health Integration will serve vulnerable children, youth, adults, seniors and families across the state of Nevada, with a focus on:
- Low-income individuals
- Individuals experiencing or at risk of homelessness
- Individuals with chronic health conditions
- Individuals with behavioral health or Substance Use Disorder (SUD) needs
- Justice-involved populations
- Rural and frontier populations with limited access to care
- Individuals facing multiple social and economic barriers
Our Mission
Harmony Health Integration Community Health Worker Services exists to bridge the gap between healthcare systems and the communities they serve by deploying trusted Community Health Workers who walk alongside individuals at the intersection of need. We are committed to advancing whole-person care by addressing social determinants of health, strengthening care navigation, and ensuring that no one is left to navigate complex systems alone. Through culturally responsive engagement, coordinated partnerships, and measurable outcomes, we empower individuals to access care, sustain wellness, and build stability—while helping healthcare partners improve outcomes, reduce costs, and deliver care with dignity and compassion.
Why We Exist
Healthcare is often fragmented, leaving the most vulnerable populations to navigate a complex maze of medical and social services on their own. Many individuals face barriers such as lack of transportation, housing instability, food insecurity, and limited health literacy, which directly impact their health outcomes.
Harmony Health Integration was founded to be the missing link. We recognize that true health extends far beyond the walls of a clinic. By addressing the social determinants of health, we help individuals achieve stability and wellness.

We envision a future where Community Health Workers are recognized as essential infrastructure—trusted bridges that ensure every individual, regardless of circumstance, has equitable access to care, support, and opportunity.
