Harmony Health Integration will deploy Community Health Workers (CHWs) as the central workforce to bridge the divide between clinical systems, public health infrastructure, and community-based support services.
CHWs are uniquely positioned to:
- Build trust within communities
- Understand lived experiences
- Navigate complex systems
- Provide culturally responsive support
Through strong partnerships with federal, state, and county public health agencies, healthcare providers, and foundations, Harmony Health Integration will function as a coordinated access point that connects individuals to the full continuum of care.
