Harmony Health Integration (“Harmony Health Integration,” “HHI,” “we,” “us,” or “our”) is committed to providing a welcoming, respectful, accessible, and inclusive environment for everyone who interacts with our organization.

HHI provides its programs, services, education, outreach, referrals, navigation assistance, and community resources without unlawful discrimination.
No person will be denied participation in, excluded from, or treated unfairly in an HHI program because of a protected characteristic.
Certain programs may have neutral eligibility requirements based on factors such as:
Geographic service area;
Age requirements;
Funding restrictions;
Program capacity;
Income guidelines;
Insurance or benefit requirements;
Clinical or social-service criteria established by an independent provider;
Grant or government-program requirements.
Applying legitimate and consistently administered eligibility requirements does not constitute discrimination.
HHI does not guarantee acceptance into a program, eligibility for benefits, or the availability of a particular service or referral.

HHI will notify the individual in advance if a requested service is unavailable or if another appropriate communication method must be used.
HHI prohibits discriminatory harassment and abusive conduct by or against employees, volunteers, participants, clients, visitors, contractors, partners, and members of the public.
Prohibited conduct may include:
Slurs, insults, threats, or demeaning comments;
Unwanted sexual conduct or harassment;
Mocking a disability, accent, culture, religion, or identity;
Refusing to work with or serve someone because of a protected characteristic;
Intimidation or retaliation;
Displaying discriminatory or hateful material;
Repeated intentional misuse of a person’s name after correction when done to harass;
Interfering with an approved accommodation;
Discriminatory treatment in referrals, scheduling, participation, discipline, or access to resources.
HHI may take reasonable corrective action when conduct violates this policy or creates a threatening, hostile, or unsafe environment.
HHI is committed to equal opportunity in recruitment, hiring, assignment, training, compensation, promotion, discipline, termination, volunteer placement, leadership opportunities, and other employment or volunteer practices.
Decisions will be based on legitimate qualifications, performance, organizational needs, applicable funding requirements, and other lawful considerations.

The program or service involved;
Any witnesses or supporting documents;

A complaint should be submitted as soon as reasonably possible so that HHI can conduct a meaningful review.
HHI will provide reasonable assistance to a person who needs help submitting a complaint because of disability, language, literacy, or another accessibility need.
Upon receiving a complaint, HHI may:
Confirm receipt of the complaint;
Request additional information;
Review relevant documents and communications;
Speak with the complainant, witnesses, and involved personnel;
Determine whether this policy was violated;
Take reasonable corrective or preventive action;
Provide a written or verbal response when appropriate.
HHI will seek to handle complaints promptly, fairly, and as confidentially as reasonably possible. Complete confidentiality cannot be guaranteed when information must be shared to investigate the complaint, protect safety, or comply with law.
Possible corrective actions may include:
Staff or volunteer training;
Changes to procedures;
Providing a reasonable accommodation;
Reconsidering a decision;
Reassigning personnel;
Corrective discipline;
Ending a volunteer, contractor, or partnership relationship;
Other appropriate remedial action.
A complainant who disagrees with the outcome may submit a written appeal to [Executive Director, Board Chair, Compliance Officer, or other appropriate person] within [15 or 30] calendar days after receiving the decision.
The appeal should explain why the complainant believes the decision was incorrect or incomplete and identify any additional information that should be reviewed.
The appeal decision will be final within HHI’s internal complaint process.
HHI prohibits retaliation against anyone who:
Reports suspected discrimination;
Requests an accommodation;
Participates in a complaint investigation;
Supports another person’s complaint;
Raises a good-faith accessibility or civil-rights concern;
Declines to participate in discriminatory conduct.
A knowingly false complaint made with malicious intent may violate HHI policy. However, a complaint will not be considered false merely because it cannot be proven or because HHI reaches a different conclusion.
Suspected retaliation should be reported immediately to the Non-Discrimination Coordinator.
Using HHI’s internal complaint process does not prevent a person from contacting an appropriate government agency.
Depending on the circumstances, a complaint may be filed with agencies such as:
The U.S. Department of Health and Human Services Office for Civil Rights;
The U.S. Department of Justice;
The Nevada Equal Rights Commission;
Another federal, state, or local agency with jurisdiction.
HHI does not provide legal advice concerning complaint deadlines, available remedies, or which agency has jurisdiction. Individuals may wish to consult an attorney or appropriate advocacy organization.
HHI may provide non-discrimination, cultural-responsiveness, disability-access, language-access, harassment-prevention, and complaint-handling training to appropriate personnel.
Employees, volunteers, contractors, and representatives are expected to cooperate with complaint reviews and comply with corrective measures.
Harmony Health Integration may update this policy to reflect changes in its programs, operations, organizational commitments, or legal obligations.
The current version will be posted on the HHI website with the effective date shown above.
