These Terms of Use constitute an agreement between you and Harmony Health Integration (“Harmony Health Integration,” “HHI,” “we,” “us,” or “our”) concerning your access to and use of harmonyhealthintegration.org and any webpages, forms, resources, content, or online services that link to these Terms.

Please read these Terms carefully. By accessing or using the website, you agree to be bound by them. If you do not agree, do not use the website.

1. About Harmony Health Integration

Harmony Health Integration is a Nevada nonprofit organization that supports community health integration, Community Health Workers, healthcare-system navigation, health literacy, outreach, engagement, case coordination, workforce development, community resources, and related activities.

The website provides general information about HHI, its mission, services, programs, resources, events, volunteer opportunities, partnerships, and ways to provide support.

2. Informational Purposes Only

Website content is provided for general informational, educational, and community-resource purposes.

The website does not provide:

• Medical diagnosis or treatment;

• Mental-health diagnosis or treatment;

• Emergency services;

• Legal advice;

• Financial advice;

• Insurance advice;

Community Health Workers and resource navigators may provide education, advocacy, referrals, outreach, coordination, and assistance navigating systems. Unless expressly stated otherwise, they do not diagnose medical conditions, prescribe treatment, provide legal representation, or replace licensed healthcare professionals.

5. No Guarantee of Services, Referrals, or Results

HHI will make reasonable efforts to provide accurate information and appropriate support, but we do not guarantee:

• Acceptance into a program;

• Eligibility for benefits;

• Availability of a provider or resource;

• Appointment availability;

• Funding or financial assistance;

• Successful placement or referral;

• Quality of third-party services;

10. Donations and Payments

You represent that:

• You are authorized to use the payment method provided;

• The payment information is accurate;

• The donation is lawful;

• The funds are not derived from unlawful activity.

Unless otherwise stated, donations are made to support HHI’s charitable mission and may be used where needed most.

A restricted donation will be treated as restricted only when HHI expressly agrees in writing to the restriction. If a designated program becomes unavailable, impracticable, fully funded, or inconsistent with legal requirements, HHI may contact the donor or use the funds in a manner reasonably consistent with the charitable purpose, as permitted by law.

Donations are generally nonrefundable. HHI may correct duplicate transactions, processing errors, or unauthorized payments when verified. Refund requests should be sent promptly to [donation contact email].

HHI does not provide tax advice. The tax treatment of a donation depends on applicable law and the donor’s individual circumstances. Donors should retain their acknowledgment and consult a qualified tax adviser.

11. Volunteer and Event Participation

Submission of a volunteer form or event registration does not guarantee acceptance or participation.

HHI may establish:

• Photography or media releases;

• Event-specific rules.

HHI may deny or end participation when reasonably necessary for safety, program integrity, legal compliance, or organizational needs.

Volunteers are not employees merely because they participate in an HHI volunteer activity. Any employment, contractor, internship, or stipend arrangement must be established through a separate written agreement.

12. Testimonials, Photographs, and Submitted Content

HHI will not intentionally publish a person’s private health story, testimonial, photograph, video, or identifying information without appropriate permission.

When you voluntarily submit content for publication and sign or otherwise accept an appropriate release, you may grant HHI permission to use that content as described in the release.

Submitting information through a general contact form does not, by itself, give HHI permission to publicly disclose your private health information or identify you in promotional materials.

HHI should obtain permission before displaying a third party’s logo or describing the organization as a formal partner.

15. Intellectual Property

Unless otherwise indicated, the website and its original content—including text, design, graphics, logos, photographs, videos, downloadable materials, and arrangement—are owned by or licensed to Harmony Health Integration and are protected by applicable intellectual-property laws.
