Harmony Health Integration—A statewide healthcare ecosystem platform and communication hub connecting Community Health Workers (CHWs), healthcare systems, community members, providers, advocates, and strategic partners—has been established at a critical moment in Nevada’s healthcare and social services landscape. While the state continues to grow and diversify, too many individuals—particularly those in vulnerable and underserved communities—remain disconnected from the very systems designed to support their health, stability, and well-being.
Across Nevada’s High-Density Urban Core, Mid-size Growth Counties, Small Rural Counties, and Frontier/Ultra-Rural regions, access to care is uneven, fragmented, and often difficult to navigate. For many residents, especially adults facing economic hardship, chronic illness, behavioral health challenges, or housing instability, the journey through healthcare and social services is not just complex—it is overwhelming.
