Harmony Health Integration recognizes that health insurance is often the gateway to better health outcomes and a more stable life. Through a whole-person care approach, Community Health Workers help identify barriers, assess individual needs, and connect people to Medicaid, Medicare, marketplace plans, disability benefits, and other entitlement programs. By helping individuals obtain healthcare coverage and access essential services, we address both physical and mental health needs while uncovering related challenges involving housing, food security, transportation, employment, and financial stability.
